5 Wishes and How to Complete the Information

Presented by the Community Coalition for Life Completion Conversations and the Sierra Vista Regional Health Center Hospice. This presentation is designed to help individuals understand what end of life care is and how to approach making the difficult decisions associated with end of life care. Presenters will also help individuals plan for end of life care and help them to feel more comfortable when discussing this subject with family members and physicians.
